Undergraduate Studies Petitions

The Undergraduate Studies Online Petition is an online form that permits students to complete the following petitions:

  • Re-Admission
  • Late Withdrawal
  • Late Add 
 Student Instructions
  1. Meet with your academic advisor and review the requirements of each form. 
  2. Prepare the required information and documents for the petition you plan to complete. 
  3. All students are required to submit a Student Narrative.
    1. A student must attach a TYPED narrative that clearly and concisely states their case and the extenuating circumstances for which they are petitioning. Students may use 'Additional Supporting Documentation' to add any items they feel are important for the committee to review that may assist them in their decision process.
  4. Make a note of the first and last name of the advisor you are working with regarding your petition. 
  5. After you have met with your academic advisor you will need to complete an . 
  6. You must complete your application accurately and honestly. 
  7. The committee meets 1 week after the submission deadlines listed below.
  8. The committee will inform you via email of their decision typically 24-48 hours after the committee meets.

Summer 2024 Petition Submission Deadlines:

  • June Petition Submission Deadline: June 10, 2024, at 5:00 p.m. 
    • Meeting Date: June 17, 2024

 

  • July Petition Submission Deadline: July 08, 2024, at 5:00 p.m. 
    • Meeting Date: July 15, 2024

*Please note that you may only complete one petition at a time and that incomplete petitions will not be considered by the committee.
 

Petition Requirements

Re-Admission

  • A student must:
    • Include the full name of the advisor you are working with regarding the petition.
    • Acknowledge that they have been out of 老澳门六合彩官方开奖 for more than 2 semesters
    • Disclose if they have ever pled guilty or been convicted of a criminal offense, currently have charges pending against you, or have had charges pending at any time.
    • Disclose if they have ever been dismissed, suspended, expelled, placed on probation, or otherwise involuntarily separated from any other college, university, or high school, or withdrawn to avoid such involuntary separation for a NON-ACADEMIC reason.
    • List all colleges, universities or institutions of higher education since you stopped attending 老澳门六合彩官方开奖
    • Complete a Student Narrative
    • Sign the Petition
       

Late Withdrawal

  • A student must:
    • Include the full name of the advisor you are working with regarding the petition.
    • Acknowledge that they are aware of a one year limit for late withdrawals, except for DOCUMENTED circumstances.
    • Provide details of the class or classes they would like a late withdrawal
    • Answer whether or not they received financial aid for the semester they are asking for a late withdrawal
    • Complete a Student Narrative
    • Sign the Petition
       

Late Add

  • A student must:
    • Include the full name of the advisor you are working with regarding the petition.
    • Acknowledge that they are aware that they need instructor support for each course they intend to add prior to completing a petition.
    • indicate which term and class(es) they would like to add (Ex. COMM 200, PSY 101, etc.).
    • Attach their completed ' Undergraduate Late Add' form for the course(s) you would like to add.
      *Note that if you have more than one form, you are asked to combine them into a single file or PDF. You will be required to upload this file to the petition.
    • Answer whether or not they received financial aid for the semester they are asking for a late withdrawal
    • Complete a Student Narrative
    • Sign the Petition

The Undergraduate Studies Petition Committee is comprised of various Undergraduate Studies staff and administrators that meet on a monthly basis to  evaluate completed petitions.
